In this article, you will gain access to the Revoke User Access Request form, learn how to fill out the form properly, and what to expect once the form has been filled out. The Revoke User Access Request form is used to revoke access to CareCar HQ and other CareCar corporate systems. The information below is needed to confirm, process, and track access in accordance with CareCar's Information Security Policies.

Fields and Expectations
- Email Address
- Email address of the person filling out the form.
- Subject
- Input "Revoke User Access for USERS NAME"
- Description
- Input any information outside of the form that we may need, if there is none please copy the subject into the description.
- Effective Date
- The date on which the user's access needs to be revoked.
- Requester Name
- Name of the person asking for access to be revoked for the user.
- Requester Title
- Title of the person asking for access to be revoked for the user.
- Requester Email address
- Email address of the person asking for access to be revoked for the user.
- Requester Phone number
- Phone number of the person asking for access to be revoked for the user.
- User Name
- The name of the person whose access should be revoked.
- User Email address
- The email address of the person whose access should be revoked.
- System Access
- This is a multi-select drop-down, please choose as many items that the user needs access revoked on.

Bulk Access Requests
In some circumstances, you may need to revoke access for more than one user at a time. If that is the case, you will want to email helpdesk@carecar.co with an attachment with all of the user's names and email addresses that need access revoked.
Expectations after submitting the form
After submitting your request, the IT team will revoke access as requested as soon as possible per the Effective Date. Note, in the case of urgent revocation events, please indicate in the Comments and reach out to the IT team on Slack.
