In this article, you will gain access to the User Access Request form, learn how to fill out the form properly, and what to expect once the form has been filled out. The User Access Request form is used to request access to CareCar HQ and other CareCar corporate systems. The information below is needed to confirm, process, and track access in accordance with CareCar's Information Security Policies.

Fields and Expectations
- Email Address
- Email address of the person filling out the form.
- Subject
- Input "User Access Request for USERS NAME"
- Description
- Input any information outside of the form that we may need, if there is none please copy the subject into the description.
- New User checkbox
- Check this box if the access request is for a new user.
- Effective Date
- The date on which the user needs access to be granted.
- Requester Name
- Name of the person asking for access on behalf of the user.
- Requester Title
- Title of the person asking for access on behalf of the user.
- Requester Email address
- Email address of the person asking for access on behalf of the user.
- Requester Phone number
- Phone number of the person asking for access on behalf of the user.
- User Title
- The title of the person who needs access.
- User Email address
- The email address of the person who needs access.
- User Phone number
- The phone number of the person who needs access.
- System Access
- This is a multi-select drop-down, please choose as many items that the user needs access for.
- System Access roles
- For all items selected add a line item in this format:
- Software platform - user permission
- Alternatively, input an existing user's permissions to copy.
- For all items selected add a line item in this format:
- Is the identified access and level of access required for this employee to carry out his or her job duties, functions, and responsibilities?

Bulk Access Requests
In some circumstances, you may need to request access for more than one person at a time. If that is the case, you will want to email helpdesk@carecar.co with an attachment with all of the users that need access.
Expectations after submitting the form
After the form has been submitted, it will be sent to a designated approver for review and approval. Once this is complete, the IT team will implement the requested changes, including sending email login information to the specified external email and invitation links to the requested systems. Please allow 24-48 hours for this process.
